Access to BIXS via a Third Party

Why fill out the BIXS Third Party Authorization and Consent form?
Some producers may access BIXS through another individual, company, or program and not directly themselves; these are known as “Third Parties”. The BIXS Third Party Authorization and Consent form is required for BIXS to comply with federal privacy and personal information protection legislation. This form authorizes the exchange of personal information and animal information between the Third Party you appoint to act on your behalf, BIXS and the Canadian Livestock Tracking System operated by CCIA. The Third Party must be approved by you to act on your behalf with the CCA-BIXS. Some producers, particularly those without computers or computer experience, those with Internet browsers or overall systems requiring upgrades or those on very slow dial-up Internet connections, may find the third party route to access BIXS more convenient and efficient. Some persons and organizations may see a benefit in acting as a Third Party Service Provider to a number of BIXS participants. A current list of Third Party servicing affiliates to the CCA-BIXS include:

Pfizer Animal Health Canada - Pfizer Gold Plus program.

How to fill out the form

CCA-BIXS Third Party Provider Authorization Application form

Producers must authorize an individual, company or program to act on their behalf as the Third Party to report their information/data to the CCA-BIXS system. You, the producer, complete the top segment of the application noted as 'Producer Contact Information'. Your Third Party delegate individual company or program information must be entered under the Third Party (Delegate) Contact Information section. Both parties, producer and delegate, must then agree to the conditions of the Third Party authorization process by signing and dating the application form. Completed forms must be returned to the CCA at the contact information provided at the bottom of the application form. Until the CCA is in receipt of the completed Third Party authorization application form and authorization is granted, no information can be reported by your delegate on your behalf, so complete the form and mail or fax it to the CCA in a timely fashion based on your information/data submission plans. From time to time the CCA maintains the right to revise, edit or update the forms related to Third Party Authorization and you will be notified of such and asked, if necessary, to update your authorizations.
